As the basis for my application for membership in Long Beach Friends Church, I affirm the following statements.

1. I have accepted Jesus Christ as my Lord and Savior.
   John 1:12-13, Romans 10:9, John 3:16



2. I accept the Bible as the inspired word of God. It is the highest and only God-given record of the teaching which Christians must accept.
   1 Timothy 4:13, 2 Timothy 3:16, 2 Peter 1:20-21



3. It is my highest purpose to devote my life to serve Jesus Christ above all other callings and to follow his teachings in all that I do in my home, family, church, work, and social life. At all times, I will seek to do what Christ would do if he were in my place.

4. I seek, through my service to Jesus Christ and my involvement in the Long Beach Friends Church to grow in the knowledge of Christ and to be changed by God into the likeness of Christ. I agree to walk in the light of holiness, forsaking all uncleanness of the flesh and spirit.

5. I will be faithful and loyal to the purposes of Long Beach Friends Church and the larger church of Jesus Christ. I will cooperate peacefully in the work of the church, promoting love and unity in my relationships with those in the church. I will avoid causing controversy inside and outside of the church. I will participate faithfully in the responsibilities and ministry of the church with my time, talents, and financial resources. I will give priority in my life to God, his purposes, and his work in the world as a member of this local body of Christ.

In addition, I am willing to accept the Faith and Practice of Evangelical Friends Church Southwest as a framework within which to accomplish God's purposes in unity with the body of believers known as the Long Beach Friends Church and the group of churches known as Evangelical Friends Church Southwest. I do hereby make application for membership in the Long Beach Friends Church.
